Notes Formatting Issue

Empathia provides a variety of personalization settings that allow users to customize the format of their notes. However, sometimes users may inadvertently enable a setting that significantly changes the appearance of their notes. This guide will help you identify and correct any formatting issues caused by these settings.


1. "Simple SOAP" Setting

The "Simple SOAP" setting in the Note Settings under Personalization controls the structure of your SOAP notes.

  • When this setting is ON: SOAP notes are generated in a simplified structure with no sub-sections, appearing as one large block of text.

  • Issue: If your SOAP notes are appearing in a big chunk without sub-sections, you may have accidentally turned on the "Simple SOAP" setting.

  • Solution: To restore the sub-sections, navigate to your Personalization settings and turn off the "Simple SOAP" option.

2. "Assessment and Plan" Setting

The "Assessment and Plan" setting in Note Style under Personalization dictates how the Assessment & Plan section is organized.

  • When set to "By Problem": The Assessment & Plan section is merged into one and divided by each problem listed.

  • Issue: If you prefer to have separate sections for Assessment and Plan but find them merged, this setting might be set to "By Problem."

  • Solution: Change the settingto "By Section" to separate the Assessment and Plan into individual sections.

3. "Multi-problem Encounters" Setting

The "Multi-problem Encounters" setting in Note Style under Personalization affects how the HPI (History of Present Illness) is organized in your notes. 


Issue: If your HPI sections are not organized as you expect, check the Multi-problem Encounters setting to see if it's set to "Chronological," "Problem Focused," or"Multiple Notes." Adjust the setting to match your preferred format.

  • Chronological: All HPIs aregrouped together in chronological order.

  • Problem Focused:HPIs are separated based on different problems.

  • Multiple Notes: Generates mini SOAP notes for each individual diagnosis and assessment mentioned.


Important Note on Changes:

Please note that when you change any of these settings, the new format will only apply to notes generated afterward. Existing notes will retain their original format unless you regenerate them.
