Copy-and-Paste Formatting Issue

If you encounter formatting issues when copying notes into your EMR, follow the steps below to address the problem and ensure your notes appear correctly.


First of all, test it out if your EMR supports rich text formatting, i.e. bold, italic, etc.  



If it does, please use the following options to keep all the formatting in place when you transfer the notes. 

Below are some common options depending on which EMR you are using


1. Click "Copy All" from SOAP or the Consult Letter page on the web-based clinician portal (www.care.empathia.ai), or Chrome Extension. This button is designed to format notes according to each EMR's requirements.


Option 1: Try the keyboard shortcut to paste the formatting 


For PC users:  use "CTRL+SHIFT+V” 

For Mac users: use “Command + Shift + V” 



Option 2: Try "Paste and Match Style"



Option 3: Try "Rich Text Note"

Some EMRs have special formatting settings, for instance, Plexia has an option for "New Rich Text Note"



If your EMR doesn't support rich text formatting, 


Try "Paste as Plain Text"

For those using EMRs in the Chrome browser, we recommend trying Google's "Paste as Plain Text" feature. This feature removes all formatting and pastes the content as plain text, which can help mitigate formatting issues.


To use this feature:

  • Right-click in the text field where you're pasting the notes.
  • Select "Paste as plain text" from the context menu.

This simple method can often resolve formatting problems by stripping out any unnecessary formatting that might be causing issues.




For EPIC users

Here’s an example workflow for using Epic Mobile-HAIKU with Empathia:
  1. Use Epic-HAIKU on the mobile app.
  2. Use Empathia’s app to create your note, then copy directly from Empathia.
  3. You can choose a specific template (we recommend creating one for “AI Scribe”).
  4. Use Wildcards in Haiku to create a block, allowing you to paste the Empathia note into the wildcard.
    • Note: This will remove any formatting.

  1. What is your EMR? 

    1. Each EMR has different formatting, style, and requirements. 

  2. Determine the Scope of the Issue

    • Does the formatting issue occur with all notes, or is it limited to a specific type of note? i.e. SOAP, Consult Letter, Referral letter, others?
  3. Recent Changes in Your EMR

    • Have you recently changed your EMR, or has your EMR been upgraded recently?
  4. Provide a Screenshot

    • Please provide a screenshot of the formatting issue as it appears in your EMR. This visual reference will assist us in diagnosing the problem more effectively.



In the meantime, using Chrome's "Paste as Plain Text" feature may offer a temporary fix.
