How to Get Patient Informed Consent

Created by Celine Lin, Modified on Fri, 28 Mar at 3:24 PM by Nora Manchester

We offer patient consent templates and built-in disclaimers to streamline the documentation process and ensure compliance with legal and ethical standards in healthcare. These tools help clinicians quickly obtain patient consent, which is essential when using AI-driven technology, especially for tasks like recording consultations.

Common processes to obtain patient consent

1. Get written consent

2. Obtain verbal consent

  • We provide a sample script for your reference.

3. Include "patient consent statement" in notes

You can customize your patient consent notice to your preferred phrasing and this will be automatically embedded in your generated documents, such as SOAP notes and consult letters.

To enable this, go to "Note Style" and toggle on "Patient Consent." This will add a disclaimer in your note, confirming that informed consent was obtained. When you have toggled on "Patient Consent" you will see "Patient Consent Text" box where you can customize the wording. (See the visual representation below for reference.)

Default text: "Informed consent was obtained from this patient for Empathia AI-assisted ambient charting. Accuracy of the note has been verified by your clinician."

Empathia AI Patient Consent Templates

Under the “Patient Consent” tab in the clinician portal, we have 3 template consent forms that you can choose from to have the patient sign.

1) We have a concise consent form as seen below.

 2) We also have a detailed consent form as seen below in case patients require more detail regarding consent.

3) We also have a verbal consent form as below.
